Thank you for participating in our DIY Body Care Crafting Class. Your safety and satisfaction are very important to us. Please read the following waiver carefully. By signing this document, you acknowledge and accept the potential risks involved.

1. Assumption of Risk – Use of Materials and Tools

I understand and acknowledge that this class involves the use of glass containers, bowls, tools, and other equipment that may break or become damaged during use. I recognize the risk of cuts, injuries, or other harm resulting from accidental breakage or mishandling of such items. I voluntarily assume full responsibility for any risk of injury or damage resulting from the use of these materials.

2. Assumption of Risk – Product Customization and Spoilage

I understand that the class allows for the customization of body care products with a variety of ingredients. I acknowledge that:

• The addition of ingredients not supplied or recommended by the instructor may affect the stability, safety, or shelf-life of the final product.

• Improper handling or storage after the class may lead to product spoilage or degradation.

• It is my responsibility to store and use my customized products safely and in accordance with instructions provided during the class.

I voluntarily accept all risks associated with modifying the base formulations, including the potential for allergic reactions, contamination, spoilage, or ineffective results.

3. Release of Liability

In consideration for being allowed to participate, I hereby release and hold harmless Body Barista (beauty confidential co), its owners, instructors, agents, and affiliates from any and all claims, liabilities, damages, or costs arising out of or related to:

• Injuries caused by broken glass or tools

• Reactions, spoilage, or harm resulting from ingredient customization

• Improper handling, storage, or use of products created in the class

4. Medical Concerns

I affirm that I do not have any known allergies or medical conditions that would prevent me from safely participating in the class. If I do, I acknowledge that I am participating at my own risk and have consulted with a medical professional if necessary.

5. Photography & Media Release

By signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms outlined in this waiver.
